VENDOR HOLD HARMLESS AGREEMENT:
I, the undersigned, hereby release and forever, discharge GEORGIA MUSHROOM FESTIVAL, Inc. and RACCOON CREEK MUSIC PARK, their respective officers, directors, employees, sponsors, agents, representatives, successors, or assigns, severally and jointly, of and from any and all liability, claims, actions, and possible causes of action which may accrue to any of said parties from every and any loss, damage, and/or injury (including death) that may be sustained while participating in the GEORGIA MUSHROOM FESTIVAL or being a guest at RACCOON CREEK MUSIC PARK.
- A signed application constitutes a contract to follow all rules and regulations.
- Failure to follow the rules and procedures may result in removal from this event.
RULES:
- PIPES: Functional glass or clay art is allowed if for tobacco use only.
- WEAPONS:
- Safety Notice: We are a weapon-free festival. In our commitment to providing a safe and secure environment for all festival-goers, the GEORGIA MUSHROOM FESTIVAL has adopted a new safety policy.
- Please be advised that the possession of firearms, weapons or explosives of any kind, or any items that could be used as a weapon, is strictly prohibited within the festival premises.
- This policy is in place to ensure that everyone can enjoy the event in the safest possible atmosphere.
- We appreciate your understanding and cooperation in making this festival a secure and enjoyable experience for all.

VENDOR CERTIFICATE OF ANALYSIS (COA):
Dear Vendor! Thank you for participating in our festival!
To ensure the safety and quality of products being sold, we require all vendors to provide a Certificate of Analysis (COA) for their products.
Please follow these instructions to obtain and submit your COA:
1. Product Identification: Clearly label each product with its name and any relevant
identifiers.
2. Contact Supplier: Reach out to your supplier or manufacturer to request a
Certificate of Analysis for each product you intend to sell at the festival.
3. COA Requirements: Ensure that the COA includes the following information:
- Product name and batch/lot number.
- List of ingredients/components with their respective quantities.
- Testing results for potency, purity, contaminants (if applicable), and any other
relevant quality parameters.
- Date of analysis and expiration date (if applicable).
- Laboratory information, including accreditation and contact details.
4. Review COA: Carefully review the COA to ensure it meets our requirements and
accurately represents the quality and safety of your products.
5. Submit COA: Submit the COA for each product to the festival organizer no later
than August 1, 2024. Send the COA to georgiamushroomfestival@gmail.com.
6. Display Requirements: Display a copy of the COA for each product at your booth
during the festival. This helps to build trust with customers and demonstrates your
commitment to quality and transparency.
7. Keep COAs Accessible accessible at your booth during the festival in case they
are requested by festival staff or regulatory authorities.
